In my sub account for Best Contractor School, I created a form in the forms section, clicked publish. And gone to Funnels, inserted the form into my funnel, selected the form I created and published and in my funnel I get a grey box with a frowny face.

I've tried creating a new form, now twice, with the same process and cannot get it to work.
