Instareport automation not working

I created an Instareport automation and I've tested it a couple of times, but it's not working. I get a notice of the new lead. I also get the contact added to the pipeline. I get an email ad the account manager but the lead doesn't get an email and the report doesn't get triggered. Also, when I look up the lead details the business website, business name don't show up along with other details that I know I entered into the form. My form has almost all fields as a requirement.

I tried creating the automation under the business contact and the person contact and it looks like the issue is that the form isn't populating the required fields "Website," and Industry. I think it's keeping it from populating the report. Also, even though the form collects the person's name, email, and job title, they do not show up as an associated person for the business. If I generate the automation under a person, then the business doesn't associate with the person/contact.
