Report a bug to the DashClicks development team.
Customer/client account website form required field wasn't completed, yet the form got submitted.
The field IS set to required, ALWAYS has been. But the latest website form submission allowed the lead to skip the "required" field and submit the form. The form ALSO displays differently from every other form that has ever been submitted, refer to subject line of the email. (Please see attached screenshots)
